Because of the level of the specialist skills in AFF and investigative skills in Gumshoe, we adopted it for our Eberron game, and I even hacked AFF2 to use for our modern day Mutant City Blues game (it doesn't take too much work to tweak the powers to using 2d6 instead of 1d6)

I've ruled it that the wizard has already used their "Attacks" so just rolls normally for a defensive SKILL check, just as the fighter concentrating on the ogre would against an orc rushing him from behind.

Fire Bolt is the only obvious one that I see through a quick perusal, but there may be one or two others (although some specifically state that the result is unable to be dodged, such as Force Bolt).

I think the Dodge skill is for use against certain spells, but if not RAW or RAI, I think I'd allow for Dodge to be used instead of making an attack if that was requested by the player. As far as "Able to take a deep breath" goes, I believe it means having the opportunity to do so: if you are sudden...

Maybe add some extra "riders" on different weapons at the 7+ damage column, so say a warhammer pushes your opponent, a spear pins them, a quarterstaff knocks them prone, and a dagger inflicts on-going bleed (as they tend to suffer penalty against armed opponents).
